Registered Nurse

Role Summary:

As a Nurse, you will regularly assess, plan, implement, and evaluate client care to maximize the restoration and maintenance of the client’s physical and emotional health. You will encourage and support clients through the rehabilitation process and gently guide them to reach their goals. You will also serve as an educator to our clients and their families.

  • Responsibilities:
    Conduct physical assessments of client and provide ordered treatments and education.
  • Perform evaluation task, including vital signs and medication review.
  • Administer medication as prescribed by the client physician.
  • Coordinate with Therapy Team, After Care Coordinator, and other members of interdisciplinary team for treatment planning.
  • Collect UDS sample, ensure order is sent to lab and review results with provider and client when received.
  • Complete an accurate count at the beginning and end of each shift of all controlled substances onsite.
  • Document progress note accurately for all clients per shift.
    Complete admission and discharge assessments and documents.
  • Perform skin check and search on all admitting clients.
  • Responsible for assisting clients needed additional care related to medical needs.
  • Communicate client changes with providers and receive
    new orders.
    Participate in treatment team.

Resident Assistant

Role Summary:

Provides direct and indirect client care under the direction of Resident Assistant Manager or Designee. Provides for the personal care and comfort of residential clients. Provides a safe, clean environment. Participates in performance improvement and continuous quality improvement (CQI) activities.

Responsibilities:

  • Performs and records client vital signs, notifies nurse onsite of any changes.
  • Demonstrates the ability to identify behaviors that require intervention.
  • Perform client safety checks at scheduled time and document appropriately.
  • Assist clients who need additional assistance with ADL, meals and transferring.
  • Follows all safety rules while on the job.
  • Completes search of personal property upon admission and when required.
  • Laundry services for new clients, including box items and linen/towels.
  • Maintains client confidentiality, following HIPPA/42F and all other privacy laws.
  • Attend/Assist with offsite outings at the direction of the Recreational Therapist and ED.
  • Transport clients to and from their scheduled appointments, admission or discharge location and center events.
  • Any and all other duties assigned by supervisor or leadership onsite.

Candidate requirements:

  • High School Diploma or GED
  • Previous healthcare experience is preferred.
  • CPR and CPI Certification (can be obtained through Cedar Oaks new employee certification)
  • Valid Drivers License (for transportation)
