Co-Occurring PTSD and Substance Use Disorders

PTSD is a condition triggered by a traumatic event most associated with severe anxiety and intrusive memories. However, people with PTSD can experience many symptoms. Some of these include:

  • Flashbacks or nightmares
  • Avoidance, including avoiding thinking or talking about the traumatic event or avoiding places or activities that remind you of the event
  • Negative changes in thoughts and mood, including hopelessness, memory problems, and feeling emotionally numb
  • Arousal symptoms, such as being easily startled, sleeping difficulties, and feeling tense or on guard, irritability, and angry or aggressive outbursts
  • Self-destructive behavior and overwhelming feelings of guilt or shame

Individuals who struggle with PTSD and addiction may have symptoms that vary in intensity over time, as well as different underlying traumas from their past, such as sexual or physical abuse, car accidents, or military service. According to the VA, a substantial majority of Veterans with PTSD also meet criteria for comorbid substance use disorders, often involving alcohol. Other research has shown that individuals with PTSD and addiction have a heightened risk for other mental health challenges, including depression and anxiety.

Signs of Substance Misuse and Addiction

Many individuals with PTSD often use drugs and alcohol to self-medicate and treat their symptoms. Unfortunately, this can lead to addiction, dependence, and a worsening of PTSD symptoms that create larger problems in a person’s life.

Signs a person may be misusing or abusing alcohol or drugs include:

  • Changes in behavior, spending habits, and finances.
  • Poor work performance and lack of motivation.
  • Changes in weight and appearance.
  • Drug-seeking behaviors / becoming argumentative about substance use.
